FREEDOM OF INFORMATION ACT REQUEST FORM

The City of Peoria was incorporated in 1845. The City of Peoria is a home-rule municipality incorporated and organized under the laws of the State of Illinois for the purpose of providing its residents services including, but not limited to: police, fire, public works, recreation, building, planning, zoning and code enforcement. The City employs approximately 610 full time and 55 part time employees, and seasonal employees as needed.

Freedom of Information Act (FOIA) Request Form

Use this FOIA Request Form to submit your Freedom of Information Act request. Unless otherwise indicated, you must provide us with the following information for your request to be processed.
