City of Peoria Bodyworks License

Any business offering bodyworks (massage) services for payment must have a Bodyworks License from the City of Peoria.

Who does NOT need a Bodyworks License:

  • Athletic trainers working for a private or public school, college, university, or organized athletic team.

  • Licensed medical facilities such as hospitals, nursing homes, or chiropractic offices.

  • Massage therapy or bodywork schools providing training.

  • Salons or shops with at least ten (10) full-time employees that can show a valid registration certificate from the Illinois Department of Professional Regulation.

Application Requirements


To apply for a Bodyworks License, you must:

  • Complete the entire application form and have it notarized before submission.

  • Undergo a background check and digital fingerprinting through the Peoria Police Department. The cost is $150.00 (non-refundable).

  • Provide two passport-sized photos (1"x1.5"), front-facing, showing your head and shoulders.

  • Submit a copy of a government-issued ID (e.g., driver’s license).

  • Pay the application fee (non-refundable).

  • Include a floor plan or diagram showing the business layout.

  • Provide your business website address (if available).

  • Sample of business advertising.

  • Proof of Good Standing with the State of Illinois.

  • Copy of Lease. (If leasing a business location.)

  • Copy of Therapist State Licenses or Certificates.

All applications are reviewed by city departments for compliance.

Application Fees

For New Applicants:

  • Bodyworks is the primary service of the business - $250.00
  • Bodyworks is the secondary service of the business (less than 50%) - $100.00

For Renewing Applicants: 

  • Bodyworks is the primary service of the business - $100.00
  • Bodyworks is the secondary service of the business (less than 50%) - $50.00
